Clinical case of autoimune hepatitieis in the child from the focus of hepatities A

Sabadash Ye.Ye., Bezkaravainiy B.A. Lugansk State Medical University, Ukraine Under our observation there was a child of 14 years old. The child lives in the city where the focus of hepatitis A was observed simultaneously with our case. After the contact with a sick with hepatitis A in 3 weeks in the connection with the development of jaundice the child was admitted to the infectious department of the local hospital. The clinical diagnosis was: virus hepatitis A, severe course. The diagnosis was established on the basis of the epidemiological anamnesis (the child had a contact with a sick with hepatitis A), clinical picture, changes of biochemical parameters of blood and finding in blood anti HAV lgM. The child received treatment in the infectious department for a month. During the treatment the condition of the child worsened, the phenomena of intoxication increased, neurologic disorders (insomnia, loss of appetite) appeared. In the connection with deterioration of the general condition he was transferred to the infectious department of Lugansk regional children's hospital. The child was admitted to the hospital in a grave condition. Severity of a state was conditioned by intoxication, hyperbilirubinemia, cholestasis, cytolytic and mesenchyme inflammatory syndrome. In the biochemical analysis of blood the increase of the level of general bilirubin at the expense of connected and free fractions was observed, mostly at the expense of connected bilirubin. The increase in activity of alanine aminotransferase, thymol test, alkaline phosphatase was revealed. The analysis on markers of hepatitis revealed the presence of antibodies of class M to virus of hepatitis A in trace amounts, and the absence of antibodies to viruses of hepatitis B and C. Analysis revealed the presence of antinuclear antibodies in blood. The child received the treatment: Bed regimen, diet, plentiful drinking. Infusion therapy included: plasma, albumin, glucose. Prednisolone, contrykal and lipoic acid were injected intravenously. Vitamin C, ”Ursofalk“, the Nicotinic acid, “Enterosgel“, prednisolone, “Mesim forte”, “Glutargine” were taken orally. As a result of the treatment the condition of the child gradually improved. The intoxication disappeared; the sizes of the liver have decreased. Biochemical analyses of blood came to the norm, positive dynamics was noted at ultrasonic research as well. The child was discharged in a satisfactory condition with normalization of biochemical parameters. In 3 months after the discharge from the hospital the condition of the child was satisfactory. Biochemical analyses were within the limits of the norm however, insignificant hepatomegaly resists.
